Had a friend who was getting spurious 401 failures on his machine. Tried swapping hard drives, SCSI cards, ribbon cables, etc. Nothing seemed to fix it. And sometimes the machine would run fine for a day or two before the error appeared.
Well, he recently found the problem. It ended up being a bad power connector going TO the hard drive. Seems the female pins had gotten enlarged slightly from plugging and unplugging numerous times. Enough to the point that the power connection wasn't as solid as it should have been.
He squeezed the female pins closed slightly for a better connection and the problem was solved!
